Transcript Request Checklist

The office will need the following information to complete a transcript request, you may email this information to the school secretary:

  • Full Legal Name of Student

  • Student's Mailing Address and Phone Number

  • What Year did the Student Graduate

  • Name of Institution to Send Transcript

  • Address of Institution

  • Name of Person and Contact Information or Department to Send Transcript

  • Date Transcripts Need to be Summitted By

  • Unofficial Request or Official Request
